Mount Horeb has many events throughout the year. Applications submitted by email or online are preferred. You can also print out the application and mail to: 300 E. Main St., Mount Horeb, WI  53572. Some applications require photos and SS number/Wisconsin Seller’s Permit Number.  For applications to be complete, please read the applications thoroughly and submit the proper materials requested.  NOTE: If the link is not active, we have not posted the application.
